Qualified Social Worker - Older People's Team - Kensington & Chelsea

Our client in Kensington & Chelsea area is looking for a Social Worker to join their Older People & Physical Disabilities Team.

Your main job duties will include:

• Holding a caseload of older people and disabilities
• Assessing, monitoring and reviewing social care needs of individuals
• Working with clients with issues with hoarding and self neglect
• To carry out needs led assessments of adults within the guidelines of Community Care Act
• Care managing complex cases including Safeguarding Adults and Deprivation of Liberty
• Co-ordinating and reviewing multi-service care packages
