Care Manager - Integrated Care Network - Bromley

The Integrated Care Networks are multi-disciplinary Hubs that include GP & Health services in that area also focussing on Prevention and Delay of services in line with the Care ACT 2014.

The networks are now established enough to receive Care Management support within these hubs.

Our client are looking for two qualified Care Mangers with substantial experience in Adult Social Care that have completed their ASYE and have good knowledge of safeguarding, mental capacity and Best Interest decisions.

You will be required to work closely with the senior care manager assessing clients and carers within the care act legislation, also formulating and monitoring support plans to meet assessed eligible needs. You will also provide information and guidance for those that do not have eligible needs. You will also complete safeguarding enquires where the threshold has been met.

You will need a full driving license and access to a car.
