Qualified Social Worker Job – Derbyshire – Older Persons Team

Our client in Derbyshire is looking for a Qualified Social Worker to join their Older Persons Team. You will be working within Care Management & Review and will be responsible for assessments and care planning for older people.

Your main job duties will include:

  • Holding a caseload of older people
  • Assessing, monitoring and reviewing social care needs of individuals
  • Working with clients with issues with hoarding and self neglect
  • To carry out needs led assessments of service users within the guidelines of Community Care Act
  • Care managing complex cases including Safeguarding Adults and Deprivation of Liberty
  • Co-ordinating and reviewing multi-service care packages

You will need to have HCPC registration and a Social Work Qualification. Ideally you will have post qualification experience working with adults and/or Older People.
